Please Prepare Ahead

Previous eye exam records should be sent to the office before the visit. Complete the child history form, or ask us to mail a copy. If your child does not yet know their letters, review the children’s eye chart we provide so the exam can go smoothly.

If these steps are not done before the appointment, we may need to reschedule or refer you to another provider.

Copays for exams, glasses, contacts, and materials are due at the visit. Non-plan orders require a deposit. Children under 18 must be accompanied by a parent unless the office has the paperwork for a teen to be seen alone.
